Sounds like a see you local doctor before you leave question
The doctor in the states may not be experienced in tropical diseases

I would say go on for six months with a quinoleine treatment against malaria and every six months get a deparasiting treatment against giardiasis, and amebiasis for the next 2 years. Consult a specialist in tropical diseases and disorders.
