Angioedema refers to swelling that occurs in the tissue just below the surface of the skin, most often around the lips and eyes. It is similar to hives, but hives involve itchy red welts on the surface of your skin, whereas angioedema is a deeper swelling. Both hives and angioedema are usually caused by an allergic reaction, to either a food or medication. In rare cases, it may be a sign of an underlying condition such as leukemia or Hodgkin’s disease. In an allergic reaction, the body produces histamine, which causes blood vessels to swell. There are two basic types of angioedema – hereditary angioedema (HAE) and acquired angioedema (AAE).

Hereditary angioedema is a genetic disorder due to a deficiency or malfunction of C1 inhibitor. C1 inhibitor is part of the complement system, which is part of the immune system. In this disorder, an injury, a viral infection, or stress may trigger attacks of swelling. Acquired angioedema is caused by a consumption of C1-INH for any of a number of reasons leading to low levels of this protein. Deficiencies in this protein allow unchecked activation of the classic complement pathway and other biochemical systems.

Common symptoms of angioedema include the sudden appearance of red welts, especially near the eyes and lips, but also on the hands, feet, and the inside of the throat; burning, painful, swollen, and sometimes itchy areas; discoloured patches or rash on the hands, feet, face, or genitals; hoarseness; tight or swollen throat; and difficulty of breathing. In a form called angioedema-eosinophilia syndrome, hives, itching, fever, muscle pain, decreased urine, weight gain, and high white blood cell count also occur.

Angioedema may be mild to moderate to severe. Mild symptoms may not need treatment, while moderate to severe symptoms may need treatment. Breathing difficulty is an emergency condition. If the person has trouble breathing, seek immediate medical help.

In all four markets, Pegasys and ribavirin is the most frequently prescribed combination for first line therapy. Options for second line are limited if patients fail on current standard of care.

Hereditary angioedema or HAE is an autosomal dominant disorder. It is an inherited disease caused by low levels of the plasma protein C1 inhibitor (C1-INH), which causes uncontrolled activation of the classical complement pathway.
